If you are an inpatient

"A medical certificate shall not be issued to the patient in accordance with the Article 17 of the Medical Law by preparing a medical certificate, optometry certificate, or any other certificate unless he or she is a doctor or a dentist who engages in the business of medical treatment and conducts consultation or optometry."

However, certificates that can be issued directly are outpatient medical examination certificates and hospitalization and discharge confirmation certificates, which show only the date of hospitalization (hospitalization period) without filling in the diagnosis.

  • "For the patient’s medical record, a person other than the patient shall not view the patient’s record or make a copy of the patient’s record in accordance with Article 21.2 of the Medical Law. However, a copy can only be issued if it meets the requirements of
    Article 22 of the Medical Law and Article 13.3 (Requirements for Reading Records) of the Enforcement Regulations of the Medical Law."

    Required Documents to Request a Medical Certificate

    If you are under 17 years old, please submit your student card, passport, or youth card.

    For under14 years old, a medical certificate cannot be issued.

    Family or relative
    (Spouse and direct line ascendants/ descendants of the patient and direct line ascendants of the spouse)
    Less than 14 years old
    1. Copy of ID of the person (legal representative) who requests a copy of a medical certificate
    2. Document to confirm your family relationship, such as copy of your family relationship certificate and copy of your resident registration card
    14 years old or older
    1. Copy of ID of the person requesting copy of the medical certificate
    2. Document to confirm your family relationship, such as copy of your family relationship certificate and copy of your resident registration card
    3. Consent form signed by the patient himself
    4. Copy of the ID of the patient
    5. To submit a copy of the patient's ID card, confirm with a student card, passport, or youth card of the patient under the age of 17.
    Representative
    (Brother, sister, insurance company employee, daughter-in-law and son-in-law, etc.)
    Under 14 years old
    1. Copy of ID of the person (legal representative) who requests copy of the medical certificate
    2. Consent form signed by the legal representative
    3. Power of attorney signed by the legal representative
    4. Document to confirm family relationship between the patient and the legal representative

    14 years old or more
    1. Copy of ID of the person who requests copy of the medical certificate

    5. To submit a copy of the patient's ID card, confirm with a student card, passport, or youth card of the patient under the age of 17.

    Consent form and power of attorney shall be written by forms 2 and 3 of the attached sheet No. 9.

    (only family or relative can apply)

    4. Death certificate is excluded if the family relationship certificate or the certified copy death certificate indicates the patient’s death.
    If the patient cannot sign an autograph due to unconscious or serious illness, or injury.

    3. Medical certificate confirming that the patient cannot sign due to unconsciousness or having a serious illness or injury
    If the patient is missing

    3. Copy of the resident registration card, copy of the written decision of the court's statement of disappearance, and other documents to confirm the person missing
    If the patient is devoid of mental capacity
    1. Copy of ID of the person who requests copy of the medical certificate
    2. Document to confirm your family relationship, such as copy of your family relationship certificate and copy of your resident registration card
    3. Medical certificate of a psychiatrist proving that the patient is devoid of mental capacity
    Usage Information
    • ID card: Residence registration card, passport, driver’s license or ID card that can verify the principal’s identity and is issued by a public institution
    • Family or relative: Spouse and direct line ascendants/descendants of the patient and direct line ascendants of the spouse
    • The consent form must clearly state the consent content, date, and scope.
    • The consent form must be signed by the patient himself/herself. (Seal or thumbprint is not allowed.).
  • "In accordance with Article 15 of the Enforcement Regulations of the Medical Law related to medical records retention, the retention period of a medical certificate is three years, and a medical certificate after the retention period cannot be reissued."

    Matters to Be Attended when Issuing a Medical Certificate

    • Fee for issuing a certificate must be paid separately from the examination fee.
    • In case of a medical certificate for soldiers, you need to bring your ID card and two photos, and visit the hospital by yourself (a medical certificate cannot be issued when you do not bring the required documents)
    • Reissuance is possible at the certificate window (No. 24 ~ 26) without receiving medical treatment.

      However, it is not possible to reissue the physical or mental disability certificate, the evaluation of working ability medical certificate, an application form for extension of the number of days of medical benefits, confirmation letter for assisting devices inspection, prescription for assisting devices, oxygen treatment confirmation letter, and disabled certificate.

    • ID card: Residence registration card, passport, driver’s license or ID card that can verify the principal and is issued by a public institution

      Family or relative: The spouse and direct line ascendants/descendants of the patient and direct line ascendants of the spouse (brothers, sisters, daughter-in-law, and son-in-law are not considered to be a family because they belong to a collateral line.)

    • The consent form must clearly state consent content, date, and scope.
    • The consent form must be signed by the patient himself/herself. (Seal or thumbprint is not allowed.).

    Automobile Insurance and Industrial Accident Compensation Insurance

    Automobile Insurance (processing procedure)

    Bring the medical insurance payment guarantee of the insurance company before medical treatment or request a fax transmission (patient, guardian) → Go to window No. 5 of the hospital administration team for payment.

    Industrial Accident Compensation Insurance

    • Processing procedure: Submit three copies of application form for medical benefits and submit them to window No. 5 of the hospital administration team → Issuance of the attending doctor’s initial written opinion → Reception at the competent labor welfare corporation → After confirming approval, we process it.
    • Documents to be submitted to the Korea Labor Welfare Corporation: Copy of the initial written opinion (Korea Labor Welfare Corporation form), copy of the medical record (preliminary medical treatment), and a radiographic film (CT, MRI, etc.)
